What is Heparin?

It is classified as an anticoagulant, which is a blood thinner, that comes in the form of an oral medication or a liquid for injection or infusion. Heparin can be unfractionated heparin (UFH) or low-molecular-weight heparin (LMWH), and Heparin comes as a brand name (trade name) or generic drug. This blood thinner drug has a complex metabolism and its half life is about 1.5 hours, which gives you an idea of how long this drug stays in your system and its duration of action. Although usually used to treat blood clots, this medication can potentially help women with a history of miscarriage reduce their risk. Instead of a fertility treatment to become pregnant, this medication is a fertility treatment to protect an existing pregnancy, increasing the chance of carrying a pregnancy to term, similar to alternative drugs like Aspirin.

Why do doctors prescribe this medication?

Traditionally, doctors have prescribed this drug to help treat and prevent blood clots, in heart disease, after surgery, and other situations. However, some doctors may also prescribe this medication  to women to reduce their chance of a miscarriage. Some women experience multiple miscarriages, and in some cases the embryo fails to implant in the placenta. This can be related to thrombosis, and doctors may prescribe this  injectable drug during pregnancy in these cases.

Sometimes medical staff will use an intravenous Heparin lock as an IV flush to keep blood from clotting in a catheter, since this drug is an anticoagulant by definition. Dissolving clots helps to keep the catheters open and the fluid flowing freely. Doctors may also prescribe low-molecular-weight Heparin as a bridge when Warfarin treatment is interrupted briefly. A doctor or nurse may administer a bolus dose of this medication after a stroke, transient ischemic attack (TIA), myocardial infarction (MI) which is a heart attack,, acute coronary syndrome (ACS), or atrial fibrillation (AFIB). In the case of a sports injury or circulation problems, doctors may prescribe a Heaparin topical gel, cream, or ointment.

How Does the Drug Work?

Heparin is an anticoagulant, and its mechanism of action (MOA) is the reduction of blood clotting. The body naturally produces a substance like Heparin, and its function is to thin the blood and prevent clots from forming or growing. Sometimes during a pregnancy, a blood clot can form in the placenta. This blood clot physically blocks oxygen and nutrients from reaching the developing fetus, causing a miscarriage. In women who are prone to blood clots, using a blood thinner like this drug can help prevent this common cause of miscarriage.

When and How to Take

Your fertility doctor will give you instructions on how and when to take this medication for fertility treatments, and you should also refer to your package insert. You will usually take the drug subcutaneously (subQ/SQ route), which is an injection under the skin, or intravenously as an infusion using a pump, which is administered through a vein, but you may also receive a prescription for oral tablets or pills. You should not administer this medicine by intramuscular (IM) injection because it could cause irritation, pain, or hematoma formation. You may be told to inject a dose of 5,000 units beneath your skin twice a day to help protect against miscarriage, although this dosage may vary, especially for other medical uses. The drug injection sites could include your arm, thigh, or abdomen (stomach area). If your fertility doctor or nurse is administering Heparin intravenously, they will determine the drip protocol, therapeutic range, Heparin gtt., titration, and drip calculation using a nomogram.

What is a miscarriage?

A miscarriage is a pregnancy that ends on its own within the first 20 weeks of pregnancy, usually within the first 13 weeks, and it is the most common type of pregnancy loss. For the average woman, the chances of a miscarriage are about 10 to 25 percent, although this increases with age. A recurrent miscarriage (RM) is three or more first trimester miscarriages in a row, and this affects about one percent of couples who are trying to conceive a child. The symptoms of a miscarriage can include tissue and what looks like a clot exiting the vagina; brown or bright red bleeding, with or without cramps; a white-pink mucus; painful contractions every 5 to 20 minutes; mild to severe back pain; weight loss; and the sudden decrease in pregnancy symptoms. If the body does not expel all the tissue on its own, the doctor may perform a dilation and curettage (D&C) procedure to prevent bleeding and infection, and they may also prescribe drugs. Studies suggest that women can reduce their chance of a miscarriage by avoiding cigarette smoke, alcohol, caffeine, and stress, along with eating well, taking in folic acid and other appropriate nutrients, exercising, checking with their doctor before taking over-the-counter drugs, and avoiding environmental hazards such as radiation.
