Patients should be made aware of the potential risk of serotonin syndrome at start of treatment and dose increase.
• Anafranil should be used with caution in patients with liver problems.
• The use of Clomipramine has been associated with hematological changes such as thrombocytopenia, agranulocytosis, leukopenia, anemia, and pancytopenia.
• Surgery before undergoing any elective surgery, the use of Anafranil should be discontinued for as the duration of treatment/recovery.
• The drug should be used with caution in hyperthyroid patients and patients that are or receiving thyroid medications, so as to reduce the risk of cardiac toxicity.
• Clomipramine should be used with caution in patients with impaired renal function.
