General Information

Anafranil is the trade name of a prescription drug containing clomipramine hydrochloride (clomipramine) as the active component. Clomipramine belongs to group of drug called dibenzazepine and it is a tricyclic antidepressant used to treat obsession, phobias (irrational fears) and depression, it is also sometimes used in the treatment of muscular weakness associated with sleepiness. Active Ingredient

The actual mechanism of how clomipramine functions in treating depression, OCD and phobias is unknown. However it is thought to function by inhibiting the reuptake of serotonin (a chemical in the brain) hence stabilizing the patients.

Indications and Dosage Information

  • Adults: The recommended starting dose is 25 mg per day, this dose can gradually be increased to a well-tolerated dose of 100 mg within the first two weeks. Subsequently the dose can be increased to a maximum daily dose of 250 mg per day.

 

  • Children and Adolescents: same as with adults, the recommended starting dose is 25 mg per day and can be gradually increased to a well-tolerated dose of 100 mg or 3 mg/kg during the first two weeks and subsequently increased over the next few weeks to a max dose of 200 mg or 3 mg/kg per day.

Note: During the initial phase of titration, Anafranil should be used in divided doses and after titration, the total daily dose should be given at night.

  • This medication should be taken with food to avoid gastrointestinal side effects.

Warnings and Precautions

Contraindications

  • The use of Anafranil is contraindicated in patients who are hypersensitive to Anafranil or other tricyclic antidepressants.
  • The concurrent use of Anafranil with MAOIs intended for the treatment of psychiatric problems or within 14 days of stopping use of Anafranil is contraindicated due to the increase risk of serotonin syndrome and vice versa.
  • Do not use Anafranil in patients currently been treated with intravenous methylene blue and linezolid, it could lead to an increase risk of serotonin syndrome.
  • Anafranil is also contraindicated during a recovery period (acute) of myocardial infarction.

Precautions

  • Monitor patients for suicide risk and clinical worsening.
  • Screen patients for bipolar Disorder
  • Serotonin Syndrome: this has been reported in fluvoxamine, SNRIs and SSRIs alone and most often when administered with other serotonergic including the following tramadol, tryptophan, lithium, tricyclic antidepressants, fentanyl, St. John's Wort, buspirone, and triptans. Patients should be made aware of the potential risk of serotonin syndrome at start of treatment and dose increase.
  • Anafranil should be used with caution in patients with liver problems.
  • The use of Anafranil has been associated with hematological changes such as thrombocytopenia, agranulocytosis, leukopenia, anemia, and pancytopenia.
  • Surgery before undergoing any elective surgery, the use of Anafranil should be discontinued for as the duration of treatment/recovery.
  • Anafranil should be used with caution in hyperthyroid patients and patients that are or receiving thyroid medications, so as to reduce the risk of cardiac toxicity.
  • Anafranil should be used with caution in patients with impaired renal function.
